The Lisle Woodridge Fire District now has our application and subscriber agreement available online.

Please click this link to download the application and click this link to download the subscriber agreement.  Please fill out the application completely  The subscriber agreement needs to be filled out at the top and signed on the right-hand side.  We will fill out the left-hand side and return it to you once the radio has been installed and tested.  The Fire District fee is $66 per month, so please include a check for $792 that will cover the monitoring cost for the entire first year.
